What is Direct Access?

Nebraska is a direct access state, which means you do not need a physician’s referral to receive services from JAM Physical Therapy. However, you should check with your insurance company, as they may still require a referral or pre-authorization in order to pay for physical therapy services. Prior to scheduling an appointment, it is important to determine what your insurance will cover. I am happy to help you through this process.

What insurance carriers do you accept?

JAM Physical Therapy is a specialty clinic, so my services are more efficient and cost effective than those of non-specialty clinics in the care of sports-related injuries. I currently accept all PPO insurances, Medicare, workers’ compensation and auto insurance claims. I also accept patients on a cash-paying basis. While I may not be on your insurance company’s list of providers, I will gladly accept your insurance and provide care that could save you money in the long run through my efficiency as a specialty clinic.

Do you accept out-of-network insurance or cash-based payments?

I do accept out-of-network insurance carriers, as well as patients who would like to utilize their HSA accounts on a cash-paying basis. Although I may not be on your insurance company’s list of providers, I will gladly accept your insurance and provide care that could save you money in the long run through my efficiency as a specialty clinic.  (HSA accounts welcome; cash/credit cards only)

What should I bring to my first appointment?

Your First Visit
As the owner of JAM Physical Therapy, I pride myself on effectiveness, efficiency, promptness and compassion. I make every effort to provide same-day appointments, or next-day appointments at the very latest. Please arrive 10 to 15 minutes early for your first appointment to fill out new patient forms. There are a few things you don’t want to forget:

  • Insurance information
  • Driver’s license
  • Written physician referral (if needed)

You should wear workout clothes or something comfortable (preferably loose fitting and accessible to the injured area). For female patients, please wear/bring a sports bra if we are evaluating an injury above the waist.

Can I fill out the first appointment forms ahead of time?

Want to get ahead of the game? For your convenience, I have electronic versions of my new patient forms available below. You are welcome to print and complete to bring with you to your first appointment, or you can complete them electronically and e-mail to or fax to 402.885.8859. Completing these forms ahead of your scheduled date will save you time at your first appointment and is greatly appreciated.

What is a running assessment?

A running assessment is a comprehensive biomechanical evaluation performed on a treadmill by a therapist who is specially trained in running mechanics. Following your assessment, you will be provided with a comprehensive report that includes individual footwear recommendations, an explanation of your running mechanics and extensive corrective exercises. You will also receive a personalized CD/DVD that contains your running evaluation.

What is Functional Movement System (FMS)

FMS identifies functional limitations and asymmetries of the body and then generates a score which is used to target problems and track progress. This system is directly linked to the most beneficial corrective exercises to restore mechanically-sound movement patterns. Learn more about FMS at

How it works

Step 1: On your friend’s first visit, they need to print your name on the new patient paperwork under the “How did you hear of us?” section.

Step 2: If your friend becomes an ongoing patient, I’ll send my “thanks” in the form of a $25 gift card!
